Requesting an appointment is simple. Just click the ‘Request an Appointment’ button and choose a time that works for you! Once the appointment is requested, I will accept the appointment and email you the consent and intake paperwork necessary to begin treatment. I check for requested appointments daily Mon-Fri. If something changes you are able to schedule, reschedule and cancel all online.
